School Board Candidate Filing Period - July 14-28, 2026 (4 seats)

Form 1095-C is prepared for each employee, retiree, and COBRA participant who was offered employer-sponsored health insurance or who was enrolled in health insurance during calendar year 2025.  Under the Paperwork Burden Reduction Act, employers are not required to automatically furnish Form 1095-C unless a copy is requested.  

Active employees may access their Form 1095-C at https://plsas.bswift.com.   Inactive employees may request an electronic or paper copy of Form 1095c by contacting Terrie Schoenbauer at tschoenbauer@plsas.org.

Form 1095-C is provided for informational purposes only and is not required to file your federal tax return.
